By completing this form the person/organization requesting the use of Bethel's Downtown facilities hereby absolves Bethel Bible Church, its pastors, leadership, members, or people of any liability for personal injury to any individual resulting from the use of the downtown church facilities and agrees to be responsible for any property damage that results during the use of the facilities.
I understand that I will be subject to an additional fine of $250 if I use glitter, glittery decor, or confetti in the rental space.
Please report any damage to the church office promptly.
PLEASE NOTE: We do not allow any outside coffee or alcohol on the premise.
